Care Management

It can be difficult to find the right care and support.  Often, the individuals and families we work  with are unsure where to start, or what kind of help they need.  Most people seek support when they are going through a lot of stress, which makes it even more difficult to plan and manage your own care.   In many instances, an assessment is needed, to figure out a clear diagnosis.  In most cases, some level of care management is needed.  Through this process, our team will help you to: 

  • Understand the situation and mental health factors involved (in psychology this is sometimes called a ‘case conceptualization’)
  • Develop a clear treatment plan that aligns with your situation, including your goals and your available resources
  • Secure the best available supports.  In many instances, we are able to provide the right therapy and support within our clinic.  But, to us, the most important thing is to make sure you have the right help, so we may also recommend or refer to other professionals in our network, if that would be the best thing for your situation.
